I have an eight-page client assessment tool for my gym that is already fully designed as a static PDF. All I need is for every response area—the typed answers and the little check blocks—to become interactive while the document’s look, layout, fonts, and spacing stay exactly the same.
Interactive elements required:
• Text fields where clients currently write answers
• Checkboxes that mirror every existing tick box
Once a client finishes the form, a single “Submit” action should trigger their completed file to be emailed directly to me (a simple mailto action is fine; no server upload is necessary).
