27-Page Fillable PDF Creation

I have a 27-page PDF that’s complete in layout and wording; it simply needs to be turned into an interactive, fill-in-the-blank document. The only interactive elements required are standard text fields and checkboxes. I don’t need email, phone, or any other pattern validation, and there are no signature fields to include.

Please keep the visual design exactly as it is—fonts, spacing, and alignment must stay intact—while overlaying the form elements so users can type or tick directly in Adobe Acrobat Reader.

Deliverables
• Final fillable PDF, fully tested for functionality on both Windows and macOS versions of Acrobat Reader
• Source file (Adobe Acrobat or equivalent) in case I need to tweak a field later

I’ll check field alignment, tab order, and that every page includes only the requested text fields and checkboxes before approving the milestone.
