PDF Vaccination Certificate Amendments

I have an existing A4 vaccination certificate in PDF format that needs a handful of precise text edits and one graphic removal. The only copy I possess is the PDF itself—no source files—so the changes have to be applied directly to that document.

What must change
• Last vaccination date
• Next vaccination date
• Pet’s age
• Date shown above the vaccination certificate heading
• “Vaccinated on” date
• Remove the veterinarian’s signature graphic entirely

Everything else on the page should remain exactly where it is; spacing, fonts, and overall appearance need to look untouched to anyone viewing the final file.

Please return a clean, print-ready PDF. If you produce an editable version (e.g., an updated AI, INDD, or layered PDF) alongside the flattened file, that would be a welcome bonus.
