PDF Autofiller from Web Form
Budget: $750 – $1,500 USD
I have a previously custom-built application that's 2 parts
Step 1) A local hosted web server using IIS hosting a web form/site ...patients fill out the form using a iPad in the office by pointing to the IP address of the local machine hosting the site. ( see attached for example)
Step 2) The webserver is also a database, stores all information entered in the website then takes the I693 pdf form and fills out information inputted from Step 1 >
The form is https://www.uscis.gov/sites/default/files/document/forms/i-693.pdf
The government updates the form every so often and with that last update the Autofiller app no longer is filling out certain portions.
I need this application updated and all bug fixes corrected. I need all details captured from form to come over to the respective fields on the form.

We use adobe Acrobat not reader to work with this application.
I need a competent developer who will get the job done 100% bug-free.. The application must fill out every single field of the pdf perfectly for you to be paid out.
I will not release funds if the application is not 100% bug free.
Programmer needs to be competent in SQL , Adobe Acrobat,IIS,Programming in C+,Website Design
