Mobile-Ready Fillable Health PDFs

I have two health-related PDFs, eight pages in total, that I need converted into fully interactive forms. The end user will be completing each page on a mobile phone, so every field, button, and signature area must display cleanly and be easy to tap on a small screen. No auto-calculations are required—just reliable text boxes, checkboxes, date selectors, and digital-signature fields where they already appear in the originals.

You are free to use Adobe Acrobat, PDF-XChange, or any comparable tool, as long as the final files open instantly in standard mobile PDF viewers (iOS and Android) without forcing the user to zoom or scroll sideways.

Deliverables:
• Two optimized, fillable PDFs that mirror the exact layout and branding of the originals.
• A quick test walkthrough video or screenshots shot on a phone showing every field is reachable and savable.

Once I verify the forms behave flawlessly on mobile, I will sign off immediately.
