Interactive PDF Registration Form

I need a clean, fill-able PDF registration form that people can edit as soon as they open it. The form only has two field types—standard text fields and checkboxes—so no scripting or automatic data validation is required.

Besides the standalone, email-friendly PDF, I also want a version that can live online for real-time completion (for example, embedded on a simple webpage or shared through a public link). If you normally use Adobe Acrobat, InDesign, or similar tools to create interactive PDFs, that will work perfectly. Just be sure that both the hosted and downloadable versions behave identically: every field must remain editable and retain its formatting once the file is saved or emailed back to me.
