Interactive Fact-Find PDF Redesign

I have a one-page Fact Find that now sits in Word. I need it transformed into a smart, modern, and clean interactive PDF. The new version must contain multiple-choice drop-down lists across more than six clearly defined sections. As each field is completed, the information should flow automatically into an accompanying Word letter template, so when the form is done the letter is ready to review and send.

Here’s what I’m expecting:
• A redesigned, branded PDF that looks polished on screen and in print, with consistent fonts, colours, and spacing.
• Interactive fields—principally multiple-choice drop-downs—accurately mapped to matching merge fields in the Word letter.
• A Word letter template set up with content controls or mail-merge links so it receives the data from the PDF without manual copying.
• Simple instructions so my team can update the choice lists or add new sections later if needed.

If you have experience building form-fillable PDFs and integrating them with Word (Adobe Acrobat, PDF form scripts, or similar tools), I’d love to see examples. Turnaround is flexible but I’d like to have a first draft quickly so we can test the data flow before final sign-off.
