Fix PDF Checkbox Glitches, clean up formatting

I have an application form that was originally built as a fill-able PDF. The text fields work fine but can’t always see the toe if it goes too long, some of the check ones misbehave. The layout also feels slightly uneven from page to page.

I need someone fully comfortable with Adobe Acrobat (or InDesign if you prefer to rebuild) and Microsoft Word to bring the document back into shape. The goal is a perfectly uniform, professional-looking form where every checkbox is visible, properly aligned, and selectable on any device. Font sizes, margins, and overall spacing should be consistent throughout so users experience a smooth, glitch-free fill-in process from start to finish.

Deliverables
• A repaired or rebuilt interactive PDF of the application form with all checkboxes functioning and aligned
• A clean, editable source file (Word or InDesign) reflecting the same layout for future updates
• Quick test report or screenshots confirming that checkboxes render correctly in Acrobat Reader, Chrome, Edge, and on print

Once I confirm everything works as expected, the project is complete.
