Fillable PDF Page Addition

Potential Freelancer,
Afternoon, generally speaking, I am looking to have an "Add Page" button added to the end of the attached "FAA 6030-1 (Blank)" PDF to allow the user to add a duplicate copy of the original blank document page after the current page (in which the button is pressed). The desired result is something like what may be seen in the "FAA 6030-1 (Blank) [Expandable v.2, PMC]" PDF (however, this example is not what I need---see Requirements section below for more details).
Requirements:
1. "Add Page" button needs to be added to the bottom-left of the "FAA 6030-1 (Blank)" PDF page (as seen in the "FAA 6030-1 (Blank) [Expandable v.2, PMC]" PDF).
2. "Add Page" button should be visible when editing/filling the PDF, but not when printing.
3. Clicking "Add Page" button should result in a new "FAA 6030-1 (Blank)" PDF page being appended to the end of the existing page(s). However, the fields in this new page should all be blank/empty except for the following which should be duplicates from whatever is input in the original page:
a. "SUBJECT OF LOG" text field (in upper-right of PDF).
b. "MONTH AND YEAR" text field (in upper-right of PDF).
c. If possible: the digital signature blocks (and their associated DATE text fields) should be shared across all pages so the blocks themselves only need to be signed once each, yet affect all duplicate fields in the PDF packet.
4. Signing digital signatures in the PDF should not lock the PDF, nor prevent fields from being edited afterwards.
Like to Haves:
- When the "Add Page" button is pressed, a pop-up should appear confirming the same information and options as appears in the attached "FAA 6030-1 (Blank) [Expandable v.2, PMC]" PDF.
