Fillable PDF Form With Email Submit

I need a professionally laid-out PDF that people can open in Adobe Acrobat Reader, type directly into, and, once finished, press a single “Submit” button that automatically emails their completed form to my personal address. When the user clicks that button no data should be stored locally—just package the form data (ideally as a full PDF copy) and trigger the default mail a so the message is ready to send. The content will be health related.

I will supply the exact field list and wording as soon as we start; for now, assume the usual contact details and a few open-text responses. Please design the fields to resize nicely, keep the file size small, and make sure everything works on both Windows and macOS versions of Acrobat Reader DC.

Deliverables
• The final interactive PDF, fully tested
• Any editable source file you used (Acrobat, InDesign, etc.) so I can tweak text later
• A short note explaining how you set up the JavaScript/Mailto action for future reference

Acceptance Criteria
The form must open without security warnings, every field must be fillable, and pressing “Submit” must pre-populate a new email addressed to me with the completed form attached.
