Fillable PDF Conversion Needed

I have a scanned PDF that must stay visually identical, but I need it rebuilt as a fully interactive, fill-able document. Every spot where a user would normally hand-write should become an Adobe-compatible form element: plain-text fields, checkboxes and signature fields. No fancy rich text or custom validation—just clean, straightforward inputs that work flawlessly in Acrobat and other standard PDF viewers.

Please recreate the form over the existing layout so margins, fonts and spacing remain unchanged. Once finished, send back the editable PDF; I will test it in Adobe Acrobat to confirm all fields behave correctly and the appearance matches the original scan.
