Fillable Delivery Note PDF

I need a clean, fill-in-on-screen delivery note that mirrors the visual style of the invoice sample I’ll share, but trimmed down to only the essentials. The form must open in Adobe Acrobat (or any modern PDF reader) with type-ready fields, so we can complete it on a computer before printing and handing it over to the recipient.
Core layout
• Header with our company name and logo in the same position and font style you see in the sample.
• A customer block showing customer company name and full address.
• A simple table for each item line containing just “Item Name” and “Quantity” columns; no pricing or calculations are required.
• A clear “Received by” section at the bottom with space for printed name, signature, and date.
Key requirements
• All fields must be form fields (text boxes or signature box) and manually editable—no auto-calculations or hidden formulas.
• Final file delivered as an unlocked, fillable PDF plus the source file (InDesign, Illustrator, or Word) in case we need tiny tweaks later.
• Fonts embedded and any graphic elements kept vector so the document prints crisply on standard office printers.
