Fillable Contract PDF Recreation

I have a scanned contract sitting in a single PDF file and I need it rebuilt from scratch so the finished document looks exactly like the original but functions as an interactive, fill-in-the-blanks form.

Here is what I expect:

• The visual layout, fonts, spacing, and page breaks should mirror the scanned contract line-for-line.
• Form fields must be added for all name and contact information blocks, every signature line, and the various date and terms boxes. Each field should be clearly tagged, tab-order friendly, and allow for text entry or e-signature.
• The final deliverable is a fully editable, fillable PDF with no loss in print quality and no watermarks.

You are free to rebuild in Adobe Acrobat, InDesign, or any equivalent tool as long as the output opens flawlessly in standard PDF readers. I will provide the existing scan immediately on award and will test the finished file on both desktop and mobile before sign-off.
