Expand Fillable Areas, Edit Text, Add Layout Sections

I have an existing interactive PDF that already contains form fields, but many of those fields are too small for the amount of information users need to enter. I will provide the original file along with a marked-up version that shows, page by page, exactly which fillable areas must be enlarged.

In addition to resizing those fields, I also need a layout adjustment: new columns/sections have to be added in the spots highlighted in my notes. No other header, footer, or margin work is required—just the additional sections that I indicate.

Key points you should know:
• All form functionality must remain intact or be recreated so the final PDF stays fully fillable.
• Visual consistency matters; fonts, spacing, and alignment should match the existing style.
• Once the edits are complete, I’ll need the updated, ready-to-use PDF plus the working file (Adobe Acrobat or InDesign, whichever you use).
