Excel to Fillable PDF Conversion

I have an existing Excel sheet whose layout and styling must remain intact, but the document needs to be delivered back to me as a professionally-built, fillable PDF. Only the dropdown menus—between one and five in total—have to be interactive; everything else can stay static text.

Here’s what I need:
• Transfer the current design, fonts, and alignment exactly as they appear in Excel.
• Re-create up to five dropdown fields in the same cells/positions, making sure each one functions smoothly in common PDF readers (Adobe Acrobat, Preview, etc.).
• Embed the allowed values for each dropdown exactly as they appear in the spreadsheet.
• Return a fully tested, ready-to-use PDF and the source file if you work in Adobe Acrobat, InDesign, or another tool.
