I am looking for a skilled designer and/or form developer to create a comprehensive wedding client onboarding questionnaire from scratch.

This questionnaire will be used in my wedding coordination business to collect detailed information from clients after booking. I want something that feels clean, sophisticated, elegant, and professional, while still being simple and easy for clients to navigate and complete.

The selected freelancer will:

I will provide most of the core questions and content, but I am also looking for someone who can:
• Suggest additional questions I may not have considered
