Dynamic Inspection PDF Template

I want to create a pdf template. That PDF template must be dynamic. Imagine a person is doing an inspection and during that inspection they find evidence (a cable on the floor). The person will take a photo (or several). Then they will upload these photos to a PDF that must be editable. The editable PDF must allow the person to upload one or several photos (whatever they consider appropriate). Likewise, it must have several (dynamic) fields so that the person can comfortably describe what they found in the photo. Therefore, it is an editable PDF with the ability to be dynamic in terms of creating fields for images and fields for text. The Word document already has 2 pages with some needed fields (they can remain like that) however the third page "Jobsite Images" is the section That I need to be more dynamic, (So the user can add as many pictures and text fields as desires).
