Digital Clinical Algorithm Conversion

I have a printed decision-making algorithm for diagnosis and treatment that must be reproduced exactly in a digital form. Everything—boxes, arrows, wording, and citation placement—needs to look just as it does on the page now, only sized for a standard 8" × 11" layout. Accuracy is critical because I intend to circulate the file among professional colleagues.

Please recreate the algorithm, preserving all clinical terminology and the reference I provide—“Clinical Psychopharmacology, 10th edition, John Preston Psy.D., Bret A. Moore Psy.D., James Johnson, M.D.” The finished file must be a read-only PDF so no one can inadvertently alter the content after distribution.
