I need a clean, user-friendly PDF that can be filled in electronically and saved with the data intact. All text fields, checkboxes, and radio buttons should behave exactly as they do in Adobe Acrobat or any modern browser viewer, so end users encounter no compatibility issues on desktop or mobile.
The layout is up to you, but it must remain clear and professional. Once I supply the wording and any logo files, please handle the form field tagging, tab order, and basic validation (for example, email format) so the document feels polished.
