Custom Adobe Stamp Creation

I'm looking for a freelancer to create a customized Adobe stamp for our company. The stamp should have a dropdown menu for selecting the cost department and cost month.
Requirements:
- The dropdown should include cost center codes.
- It should also include the option to select from less than 5 department names.
