Convert Nursing Form to Fillable PDF

We are converting a 24+ page nursing assessment and care plan (currently in Google Docs) into a fully formatted, fillable PDF optimized for Adobe Acrobat and iPad use. This is a structured clinical document used for initial assessments, reassessments, and supervisory visits. We do not require advanced scripting, conditional logic, or complex validation rules, but we do require thoughtful formatting and clean field architecture.

Scope includes:

- Full formatting cleanup and layout refinement of the existing document (spacing adjustments, adequate note areas, properly sized checkboxes, mobile-friendly layout improvements)
- Repeated header fields (e.g., Client Name, DOB, RN initials) that auto-populate across all pages
- Hundreds of properly grouped checkboxes
- Yes/No fields must function as mutually exclusive (radio-style).
- Multi-select sections must allow multiple selections
- Dropdown menus where appropriate (e.g., goals, teaching plans), with the ability to select multiple items within those fields when clinically necessary
- Open-ended date fields (no forced date formatting restrictions)
- Multi-signer capability on the same page (Client, LPN if applicable, RN), optimized for iPad stylus/finger signing
- Post-signature editability (document must remain editable after signatures)
- Clean, consistent Adobe Acrobat compatibility (desktop + tablet)
- Final smoke test confirming performance in Acrobat and common browser PDF viewers

The final product should be polished, intuitive for non-technical nursing staff, and ready for immediate use in the field.
