Comprehensive Wedding Questionnaire PDF

I’m building a best-in-class onboarding experience for my wedding-planning clients and need a single, fillable PDF questionnaire that walks them through every detail of their day—from the basics to the fine print.
Here’s the vision
• Format: interactive PDF that clients can save, close, and reopen without losing data (Adobe-compatible).
• Flow & polish: a clean, modern look that carries my Clutched Events logo on every page and guides users section-by-section with clear headings, short instructions, and logical progressions.
• Core sections: Personal Information, Ceremony, Reception, Wedding Party, and Vendors form the backbone. I’m happy for you to propose any additional sections or sub-sections you feel are critical so nothing slips through the cracks.
• Future-proofing: please build the file so I can later add, remove, or reorder questions while preserving fonts, styling, and field formatting.
What I’ll sign off on
1. A fully functional, branded PDF with editable text fields, checkboxes, date pickers, and drop-downs where appropriate.
2. Built-in “Save & Resume” functionality (either native to the PDF or via free Adobe Reader features).
3. Clear field naming and an unlocked master copy that allows me to edit content and duplicate formatting for new questions.
4. A quick walkthrough or short written guide showing how to make future edits.
