Branded Fillable PDF Creation

I have a completed layout and branding assets ready to go; what I need now is an interactive, fill-able PDF built on top of that design.

The file should include:
• Text fields for user-entered information
• Checkboxes for multiple-selection items
• Radio buttons where only one option can be chosen

No calculations are required—just reliable, user-friendly fields that respect the look and feel of my existing artwork and color palette. Please keep every visual element exactly where it belongs so the finished PDF mirrors the supplied design down to the pixel.

The final deliverable will be:
1. A fully functional, fillable PDF tested for Adobe Acrobat Reader and common browser viewers.
2. A quick walkthrough (screenshot or short clip) showing each field working as intended.
3. Up to two rounds of minor tweaks to ensure everything aligns perfectly with the brand.
