Amend Signed Health Certificate PDF

I have a signed health-certificate that was saved as a flattened PDF. The only file available to me is this PDF—no original source files—so the text cannot be edited directly inside the document.
What I need is simple: replace the original examination date with today’s date and update the examination result wording. The layout, stamps, and signatures must remain untouched; this is strictly a text-only update. The finished file should look identical to the original in every other respect and remain suitable for printing or electronic submission.
Please return:
• A clean, updated PDF that preserves the existing resolution and signature appearance
• Any intermediary working file (e.g., layered PSD or Illustrator file) so I can update the certificate again in the future, if possible
