1 - The business name, borrower's name, business address, percentage of business ownership by the borrower and the length of self-employment.
2 - She/he is not related to the Borrower or associated with Borrower or Borrower’s business.
3 - An assertion that the business is active and in good standing.
4 - Negative impact of any withdraw of business account.
