Payroll Summary

I would like the payroll summary to produce a report that shows all employees in alphabetical order showing all locations there worked at for the week all on one sheet. All locations have a specific number associated with them. That also needs to be on the report. I have included a sample of the report needed for the payroll company. You will see some employees worked at multiple locations per week, the name is repeated if there are multiple locations.
