Need help from a CPA in figuring out a few things

I need to have someone take a look at our numbers and help me get our balance sheet and P&L correctly setup. I have the template I have been using that I can send, but I do not think it correctly represents what we are doing dollar wise and I need help ensuring this information is always accurate.
