Accountant letter for NSW 2021 COVID-19 business grant

I began my business in January 2021 and seeking an accountant's letter for the purposes of applying for the NSW Government's 2021 COVID-19 business grant. The template for the letter is provided by Services NSW and is attached.
I use Quickbooks and can provide a copy of my profit and loss statements for the requisite periods. The report used is cash basis.
