Fillable PDF Enrollment Forms with "Submit" button

I need two interactive PDFs that parents can open straight from their inbox, complete on-screen, and return with one click. Each form must include clearly defined sections with headings and sub-headings for the three information sets we collect: the child’s personal details, emergency contacts, and any relevant medical information.

When a parent presses the built-in “Submit” button the file should automatically attach itself to an email and send back to our designated address as a flattened PDF copy—no extra steps for the user, and no external web links. Basic field validation (e.g., required phone number, email format) will help cut down on incomplete submissions.

I will provide wording for every question and our program logo so the layout stays on brand. You’ll handle:
• Designing both forms in Adobe Acrobat (or similar) with clean, kid-friendly styling.
• Configuring the internal mail script so it works reliably across common desktop and mobile mail clients.
• Testing end-to-end and sending me the final PDFs plus instructions in case we ever need to update an email address or text field.
