Please see included images - we need a digital copy of this health report that we can easily amend and send electronically. We need all the traffic light system to be drop down menu i.e. easily select red, amber or green. We need all the name and result sections easily populated. We would like the client name to automatically copy through from the title page to the header of each page. We would like the result overview section to be a drop down menu with 3 options (amendable by us) for normal result, borderline result and abnormal results to further explain the traffic light system. It should save as a PDF and be unamendable by the client on receipt.

I need a digital health assessment form designed in PDF format. The form should be user-friendly and cater to health assessment needs. Although the primary format is PDF, I am open to discussing future possibilities of having it in a web-based form or Excel sheet.
