I need two well-argued discussion posts that respond to the follow-up care questions embedded in the patient case studies I will upload. Each post must integrate current evidence—peer-reviewed scholarly articles published within the last four years—and all citations and references must follow APA style without exception.

The clinical focus spans medication management, patient education, and ongoing monitoring and evaluation for both cases, so please weave each of these threads naturally into your analysis instead of treating them as separate sections. I will also attach the grading rubric and documentation requirements; your work has to align point-by-point with that rubric.

Because the submission will be screened through Turnitin, originality is critical: no AI-generated text, no patch-writing, no plagiarism. Cite every idea that is not your own, paraphrase accurately, and keep direct quotations to an absolute minimum.
