Leadership Team Questionnaire Design

I am preparing a series of research articles and need a well-structured questionnaire that will let me gather primary data on leadership styles in business administration, with a sharp focus on how those styles affect team performance.

Here’s what I need from you:
• A complete survey instrument (digital-ready, e.g., Google Forms or Qualtrics compatible) that captures respondents’ demographics, identifies the leadership style in play, and measures its impact on team outcomes.
• Clear, validated Likert-scale items and any open-ended prompts necessary to enrich the data set.
• Logical section flow, skip logic where useful, and concise wording to keep completion time under 10 minutes.

Acceptance criteria
– Questions are grounded in current leadership and team-performance literature.
– Scale items include citations or brief rationales for validity.
– File delivered in both an editable document (Word/Google Doc) and a share-ready form link.
