Overview
Evidence-based practice (EBP) has been shown to lead to higher quality care, improved patient outcomes, reduced costs, and greater nurse satisfaction. This is known as the quadruple aim, which healthcare systems across the country are continually striving to reach. However, research suggests it can take 15 years or more for an EBP change to occur at the bedside. For example, evidence has shown that the spread of infection can be decreased when best practices in handwashing are followed; yet proper handwashing often remains below standards. Consider examples in your own professional experience where changes in practice, based on evidence, have taken time to implement. This demonstrates how important it is to be an advocate for change and engage in the challenge of evidence-based practice as an EBP champion in your organization. Remember, EBP starts with asking the good question, which should be searchable and relevant to your practice.

In this project, you will identify a nursing practice problem either within your practice setting or as a topic of interest within the literature and develop a PICOT (patient, intervention, comparison, outcome, time frame) question to address it. You will then use a systematic approach to conduct a literature review of the scholarly sources that are directly related to your PICOT question. This review and critical evaluation of the best available evidence will provide the background information on your topic, reveal a connection between the sources and your PICOT question, and reveal any gaps present in the literature as you determine how to best advocate for change.

In this project, you will synthesize the available literature and extract the themes, including a Literature Review Matrix, based on a PICOT question that you develop. Note that in previous modules, you already began to develop a PICOT question and a Literature Review Matrix. You should incorporate your work from both of these areas into your project after careful consideration of the instructor feedback you received for each. At least 10 scholarly sources from within the last five years must be incorporated in your review. Specifically, you must address the following rubric criteria:

PICOT Question: Create a specific and searchable PICOT question. The question you develop should be something that you can address using evidence. It needs to be written in the PICOT format. Also, be certain to include any relevant background information on the nursing practice problem that the PICOT question would address.
Significance: Defend the significance of your PICOT question. Begin by explaining why this change needs to happen and then discuss why EBP is the right approach to address the PICOT question. Use the following questions to guide your response.
Why is this PICOT question something that should be looked at to see if a change in practice is needed?
What is the significance of this change in nursing practice?
How would patient care, patient satisfaction, or cost be improved by taking a more focused look at this PICOT question?
Search Strategy: Develop an appropriate search strategy to address your PICOT question. You must explain your strategy and justify how it allows you to find relevant evidence. Address the following as you write.
What keywords did you use?
What are your inclusion and exclusion criteria?
What are appropriate databases to search? You must include a minimum of two.
Critical Appraisal: Appraise each source critically to determine its value. Each source is appraised on the Literature Review Matrix. As you work on Project One and compile the literature review, describe the evidence you chose when making the case for how to address your PICOT question. Then synthesize the literature reviewed, explain the themes you found, discuss how the literature supports or refutes your PICOT, and describe any gaps found within the literature. How does each source address the PICOT question? To organize this work, first complete the Literature Review Matrix Template before writing out your full critical appraisal in paragraph form. Remember, you have already begun to review five scholarly sources but will need a minimum of 10 to meet expectations. Your Literature Review Matrix Template should be included as an Appendix. The Literature Review Matrix Template is available in the Supporting Materials section below. Be certain to include the following in your appraisal for each source on the template:
Author, year of publication, and title
Purpose of the study
Population, setting, or both
Conceptual framework, model, or theory
Methodology or design
Variables studied and measured or data analysis
Findings of the study (conclusion and strength and weakness)
Analysis (level of evidence and viability of source)
Integration of Evidence: Analyze how the evidence addresses PICOT question. Synthesize and integrate the evidence from the Literature Review Matrix, extracting the common themes to support or refute the problem selected and any gaps in the literature. Your goal here is to synthesize the evidence to form a narrative or traditional literature review.
