WordPress Petition Form Fix

Our SpeakOut! Pro plugin has suddenly stopped saving new petitions on a Thrive-Theme WordPress site (thecadetfoundation.org). I need the existing petition draft—supplied in the attachment—successfully built and published within 24–48 hours.

Here’s exactly what must work:

• On /action a visitor chooses a county from a simple dropdown.
• The county-specific petition loads immediately below the selector.
• After signing, their entry is emailed to the list of officials we provide for that county.

Once the first county template is functioning, I have to be able to duplicate it quickly for the other counties myself.

I’m open to whatever solves it fastest: repairing SpeakOut!, configuring WPForms Basic, or another lightweight form solution—provided it integrates smoothly with Thrive Theme and lets us reuse the structure.

Please tackle the plugin issue or implement a fresh form, wire up the email notifications, and walk me through cloning the petition for additional counties. Time is the priority; simple, reliable delivery is all I need.
