Php Script / MySQL Database / Html Form

I am looking for a front-end form to upload csv file in mysql database. I will upload existing database only 1 filed "phone number".

So when I upload next file; it should not add duplicates. And after adding the new file; it must output csv removing the dups. Thats it.
