Build a simple Index page..

I need a simple Index page by HTML, CSS and JQuery/Java scripts..
Please don't bid if you don't have good knowledge on HTML, CSS and JQuery/Java scripts..

All details will be send in PMB, after that you need to send me a template..
After approved this template make a web page by help of HTML, CSS and JQuery/Java scripts..
I need a good work ..
I will upload milestone, but payment after end of the work..
