A Kuwait national/insured visited Al-Adnan Hospital in Kuwait in October 2020 and was diagnosed with Parkinson's disease - we would like his medical history and the exact date he was diagnosed with Parkisnon's disease.
We will provide subject's signed consent/authorization and other Hospital documents.
