Forminator Custom Product Order Form
Budget: $10 – $30 USD
I need a freelancer to build a custom product order form using Forminator inside my existing WordPress + Elementor PRO website.
This is not a full website project.
This is not about WooCommerce.
This is not about SEO, design, or anything else.
I need one thing only: a working, styled, and fully functional order form.
Requirements:
Up to 10 products selectable
Delivery method: Shipping or Pickup
Address field should appear only if "Shipping" is selected
Payment method selection: Yape, Plin, or Bank transfer
Show specific payment instructions (QR or info) depending on selected payment method
Upload field for proof of payment
Privacy checkbox before submission
All form text must be in Spanish
Form must send email notifications to admin (optionally to customer)
Clean, mobile-friendly layout
Form must be embeddable via Elementor shortcode
Do NOT offer me:
Website redesign
WooCommerce setup
SEO services
Hosting
"Website improvement suggestions"
Any unrelated service
If you try to upsell me services I didn’t ask for, you will be blocked immediately.
