DOM manupulation in JS website

You will have to show the drop down menu in a website, because if we can't select manually they can select automatically which is not granted. The problem is I can show the drop down menu bar at the time of filling the form but when I select specific item and print that form ..... The total details of that Value from the select part is missing. So you will have to find the value which will show off the total details from the storage in website backed.
Example: If you select a medical center after showing the drop-down menus. The printed results will have the details about that medical address, contact, email etc which already stored in that website. I can't connect that Value from DOM manupulation. So only expert can contact me. I'll need previous example or live example before assign the project.
