3d model, 3d printing

I am looking for someone that can take an image of a dog and create a 3d model. The model will be approximately 30cm x 30cm x 2cm tall. Attached is a rough sample of what I need. Feel free to use your artistic sense, it does not have to be extremely accurate, however, keep in mind that this model will be carved on a CNC, so character lines and recesses are more important than accuracy
