Functional Prototype & 3D Model Creation

I'm seeking an experienced 3D designer and prototyper to create a simple yet functional 3D model and prototype for me. This prototype will be used for functional testing, so it needs to be made out of plastic and have a moderate level of detail.

Key Requirements:
- Create a 3D model that can be used for functional testing
- Design a corresponding plastic prototype
- Develop molds for mass production
