Created 3d model from an image, for CNC carving onto wood.

I have included a picture of an Elk carved into wood, where the scene is raised from the background. This is the kind of carving I need to do, but with an image of a climber. I have attached the image of the climber. I need it to be based on a flat surface, but the body and the holds he is climbing on to be raised from that surface. Ignore the rope going to him. Just need the person and the holds on the wall 3d modeled so they are raised from the flat background.
