Develop a mobile phone application to take photo records.

I am after a mobile phone application that has a simple selection of a project number and to other text fields and then takes photos naming the image file with the project number text fields and date time. The images are to be stored separate to the phones photo library and uploaded to onedrive or dropbox. Once upload confirmed then image is removed from phone.
