PDF Medical Form Creation
Budget: $15 – $25 USD
I have two lengthy forms that need to be stylized to the client's branding, fonts, etc. The forms contain checkboxes, form fills. Looking for the documents to be completed in Adobe InDesign. Upon acceptance of the job, I will send a copy of the previous documents that have been made for the client. The fonts used are Adobe Creative Cloud Fonts: Proxima Nova Condensed (Must have the adobe fonts synced).
After mock up approved. We will ask for both PDF and InDesign files. The amount of pages for each form are as follows:
Document 1: 1Page
Document 2: 7 Pages but 4 out of the 7 are all the same because this is being printed on Carbon Copy Paper
