Need an Online Form Created
Budget: ₹100 – ₹400 INR
I need an online form created, where 3 team members can enter their comments and/or give score to individual line item. There are about 11-12 selections made (either yes or no) or on scale of 1-10.
Once the scoring is completed by three members of the team then it will provide the scorecard.
The freelancer can suggest the system of the see fit for the purpose.
