Adobe PDF with Certificate-based Signature Blank

I'm looking for someone to help me create an Adobe form that requires a certificate signature, just by clicking on the form's signature box. It should work with Adobe Acrobat Reader. I do not want users to have to navigate to the Certificates tools.
